Incident Response Agent: Turning Past Incident Knowledge Into Better Future Responses

Introduction

Modern digital systems support websites, applications, online services, banking platforms, healthcare systems, educational platforms, and many other important services. As these systems become more complex, production incidents are also becoming more difficult to diagnose and resolve. A small technical problem can sometimes make an application slow, unavailable, or completely inaccessible to users. When such an incident occurs, engineers must quickly identify the cause, test possible solutions, restore the service, and understand what happened.

One major challenge in incident response is that organizations often have valuable knowledge from previous incidents, but that knowledge may not be available at the exact moment it is needed. Engineers may have to search through old incident reports, logs, tickets, and documentation to find out whether a similar problem happened before. As a result, troubleshooting may begin from the beginning even when a similar incident has already been successfully resolved.

The Incident Response Agent addresses this problem by using AI agents and Hindsight memory. The idea is simple: instead of only storing information about previous incidents, the system remembers what was learned from them. When a new incident occurs, the agent recalls relevant past experiences and uses them to guide the engineer toward a possible resolution.

The central idea of the project is:

Every resolved incident becomes useful experience for the next one.

The Problem: Database Connection Timeout

Consider a real production situation where a website suddenly becomes slow or unavailable. The application is unable to connect to its database within the required time. This can affect users immediately because the application depends on the database to retrieve or store information.

When such an incident occurs, engineers need to investigate several possibilities. They may examine application logs, database logs, recent deployments, configuration changes, network conditions, connection limits, and other possible causes. They may also try different fixes before discovering the actual solution.

The difficulty increases when a similar incident has happened in the past. The organization may already have a record explaining what caused that problem, what actions were unsuccessful, and what finally solved it. However, if this information is buried inside old incident records, engineers may not immediately find or remember it.

Repeated troubleshooting creates several problems. Engineers can repeat investigations that were already performed previously. They may also try actions that had failed in the past. At the same time, successful solutions from previous incidents may not be recalled when they are most useful.

Therefore, the challenge is not simply storing incident history. The real challenge is turning incident history into useful experience.

Our Solution: Incident Response Agent

Our proposed solution is an AI-powered Incident Response Agent that uses Hindsight memory to remember previous production incidents and recall relevant experiences when a similar incident occurs.

The agent is designed to support engineers during incident investigation. When a new production problem appears, the agent first understands the current incident. It then searches its memory for similar incidents and brings relevant information into the current context.

The recalled information can include:

- The cause of a previous incident
- Actions that were attempted but failed
- Actions that successfully resolved the issue
- The final outcome and lessons learned

Instead of forcing the engineer to search manually through large amounts of historical information, the agent provides relevant experience at the point of investigation.

The new incident is also not wasted after it is resolved. Its cause, actions, resolution, and outcome can be stored as new memory. This creates a continuous learning process where every resolved incident can improve the system's ability to respond to future incidents.

How the System Works

The Incident Response Agent follows a continuous cycle:

Remember → Recall → Guide → Resolve → Learn

First, a new production incident occurs. For example, the application may report a database connection timeout.

The agent then understands the current incident by examining the available incident information. It identifies important details that can be compared with previous incidents.

Next, the Hindsight memory mechanism recalls relevant past incidents. The objective is not simply to find incidents that look similar, but to retrieve useful knowledge from those experiences.

The agent then provides guidance based on the recalled information. It can show what caused similar incidents, which actions failed, and which action successfully resolved the problem.

The engineer can use this information during the investigation and resolution process. The final result of the new incident is then stored as new memory.

This produces another cycle:

New Incident → Understand → Recall → Compare → Guide → Resolve → Store

Over time, the system can build a valuable collection of incident experiences.

What Does Hindsight Memory Remember?

A key feature of our solution is that Hindsight memory does not simply remember that an incident happened. It remembers what was learned from the incident.

The memory can be organized into four important categories.

1. Cause

The system remembers what caused the previous incident. For example, a database connection timeout might have been associated with a particular configuration problem, resource limitation, deployment change, or another identified cause.

2. Failed Action

The system remembers actions that were attempted but did not solve the problem. This information is valuable because engineers can avoid unnecessarily repeating unsuccessful troubleshooting steps.

3. Successful Action

The system remembers what actually resolved the previous incident. When a similar problem occurs, this information can provide useful direction to the engineer.

4. Post-Mortem and Outcome

The system remembers what was learned after the incident was resolved. This includes the final outcome and important lessons that should be remembered for future situations.

Together, these four types of information transform raw incident history into practical experience.

Traditional Incident Records vs. Hindsight

In a traditional approach, incident records are usually stored for future reference. When another incident occurs, an engineer may need to search through previous records manually. This can take time, especially when there are many incidents and large amounts of documentation.

The Incident Response Agent changes this approach.

With Hindsight, when a new incident occurs, the system can recall similar incidents and bring previous failed and successful actions into the current context. The engineer can therefore begin the investigation with relevant historical knowledge rather than starting completely from scratch.

The difference can be summarized as:

Traditional approach: Store the past.

Our approach: Learn from the past.

This distinction is important because the value of incident management does not come only from recording what happened. The greater value comes from making previous experience useful during future incidents.

Before and After Hindsight

Without memory, a new incident can lead to a repeated troubleshooting process. Engineers investigate the problem, try possible solutions, and may repeat approaches that were already attempted during previous incidents.

With Hindsight, a new incident can trigger recall of a similar previous incident. The engineer can see what failed before and what worked before. Past knowledge becomes part of the current response process.

In simple terms:

Without memory: Start again.

With Hindsight: Learn from what happened before.

This approach can help reduce repeated troubleshooting and make previous incident knowledge more accessible.

Continuous Learning

One of the most important aspects of the Incident Response Agent is its continuous learning cycle.

The system does not stop learning after one incident. Every resolved incident can become a new source of knowledge.

For example, suppose the agent recalls a previous database timeout incident and helps an engineer investigate the current problem. After the current issue is resolved, the new cause, attempted actions, successful resolution, and post-mortem information can be stored.

Later, when another similar incident occurs, the system has more experience available to recall.

This creates a growing knowledge cycle:

Incident → Recall → Guidance → Resolution → New Memory

As more incidents are resolved, the system can accumulate more practical experience. The goal is to make incident response increasingly informed by what has already happened.

Advantages of the Proposed Solution

The Incident Response Agent offers several potential advantages.

First, it can reduce repeated troubleshooting by recalling relevant incidents when similar problems occur.

Second, it can reuse valuable experience from previous causes and solutions. Engineers do not have to depend entirely on manually searching historical records.

Third, it supports continuous learning because the outcome of every resolved incident can become new memory.

Fourth, it provides contextual guidance by bringing previous failed and successful actions into the current investigation.

Most importantly, the system changes the role of historical incident data. Instead of being passive records stored somewhere for future reference, previous incidents become active knowledge that can support future problem solving.

Prototype and Real-World Potential

Our prototype demonstrates the concept of an Incident Response Agent built around Hindsight memory. The system focuses on the complete journey of an incident: understanding the issue, recalling relevant experiences, guiding the investigation, resolving the problem, and storing the new outcome.

The concept can be extended to real-world production environments where organizations handle large numbers of incidents. As incident databases grow, AI-assisted memory can become increasingly useful for connecting current problems with previous experiences.

The system is particularly valuable for environments where fast incident resolution is important and where teams repeatedly encounter related technical problems.

It can also help preserve organizational knowledge. When experienced engineers solve difficult incidents, their knowledge can be captured as part of the incident's memory rather than remaining only with the individuals who handled the problem.

Conclusion

The Incident Response Agent is designed around a simple but powerful idea: organizations should not just store their past incidents; they should learn from them.

Using Hindsight memory, the agent can recall similar incidents, understand what caused previous problems, identify actions that failed, recognize solutions that worked, and use those experiences to guide future incident responses.

The system creates a continuous learning cycle in which every resolved incident contributes to future problem solving. A new incident becomes an opportunity to recall previous knowledge, apply relevant guidance, resolve the problem, and create new memory.

This approach can help transform incident response from a process of repeatedly investigating problems into a process that increasingly benefits from accumulated experience.

The vision of our project can therefore be summarized in one statement:

“Don’t just store incidents. Remember how to solve them.”
